The Nurse Practitioner or Physician Assistant in the Inpatient Abdominal Transplant Coordinator position will be responsible for coordinating inpatient care for kidney and liver transplant patients. This includes facilitating effective communication between the medical, surgical, and transplant support staff, promoting continuity of care as patients are transferred between units, providing transplant related education to nursing units on an as-needed basis, promoting adherence to multidisciplinary team documentation requirements, and all other duties that will promote exceptional patient care to our patient population.
University of Utah Job ID# PRN02319B, Division of General Surgery Duties: Duties include, but are not limited to, the following:
1. Assesses the condition of abdominal transplant recipients who are immediately post transplant or who are readmitted with ongoing issues by performing physical examinations and obtaining medical histories.
2. Formulates the medical plan and prognosis based on the patient's condition. Works as a liaison between the surgical and medical teams in preparing patients for discharge and throughout the inpatient stay.
3. Actively consults with Surgical and Medical teams in regards to patient health care plans and treatment for abdominal transplant recipients as well as living donors.
4. Orders, interprets, and evaluates diagnostic tests to identify and assess the patient's clinical problems and health care needs.
5. Initiates and/or recommends medical drug therapies or other forms of related therapeutic treatment and procedures in accordance with approved protocols/policy. Qualifications: Licensure to practice as a Nurse Practitioner in the State of Utah. Nationally certified as a Nurse Practitioner as required under State Law. Certification in ACLS/BLS/NRP/ATLS may be required by your department or by privileging criteria. Demonstrated clinical competence and in-depth knowledge of specialty area; and demonstrated leadership, teaching, research and communication skills are required.
