R.N. Case ManagerPlace of performance: Draper, UTDuties and Responsibilities: Track status of all Soldiers with BH issues, liaison with units, advise on COA via eProfile referrals, gather statistical data and verify profile (if applicable), oversee scheduling of BH evaluations, ensure documentation is received for all appointments and follow-up is conducted, educate unit personnel on processes and BH issues.Case Management; resource all case managers for clinical issues, liaison with OIF/OEF Coordinator at VA, update policies and SOPs for SRP, PHA, and PDHRA, serve as POC for AGR, MACOMs, and PDHRA, review all PHAs not seen by a case manager at scheduled PHA, host Case management meetings and education as necessary, perform case management duties (fill in) during times of vacancy.Handle Special Cases as necessary; situations other case managers are unable to resolve, cover clinical issues, when clinical background is necessary to evaluate steps to clear Soldiers refer to which department, provider, etc., investigate, develop timelines, gather documentation on specific Soldier issues/cases at request of G1.Quality Assurance/Quality Improvement (QA/QI): advise QA/QI staff on current issues to be followed, assist QA/QI on developing processes for improvement, develop tools for ensuring QA/QI in case management.Assist in transition of Soldier issues from case management to Medical Hold section and coordinate with MHC to assure functional process involving Med NCOs and case management section gathering necessary documentation, etc.Assist the State Surgeon or Acting State Surgeon and State PA with addressing clinical issues/education needs of medical branch staff, resource for direction of miscellaneous issues which may arise, assist with high profile situations in case management, develop policies in concert with State Surgeons vision of medical branch duties and obligations.Coordinate with Medical Event Coordinator to establish best practices of communication between contract providers and UTARNG medical branch, assist with developing roles of Med NCOs and case coordinator/manager at contract events, resource for needs on UTARNG case management section in relationship to contract events.Attend SRP events as Case Manager, documenting medical issues, provide oversight to other case managers at SRP event, liaison between traditional providers and SRP needs, ensure SRP immunizations are mission appropriate and resource for traditional 68W and 66H staff, advise Med NCOs of expectations for clearing Soldiers for mobilization, counsel Soldiers at SRP on process for clearing, supply other case managers with spreadsheet of Soldiers medical issues. Maintain Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines.Perform other responsibilities and tasks which meet the organizational needs of the medical branch.Knowledge, Skills, Abilities, Experience: Equivalent life/work experience in related field; i.e. human resources, counseling, business program manager, social worker, LPN, RN, medical office support, administrative support).Knowledge of different Microsoft applications for the purpose of creating and maintaining database to support the medical branch.Knowledge of computer software and hardware, including applications and programming.Ability to express ideas orally, in writing or by means of flowcharts and graphic designs.Must possess analytical skills, knowledge of arithmetic, algebra, statistics, and other applications.Ability to deal tactfully and diplomatically with others; function under pressure such as time limitations or intense public interest.Contractor must reside in Utah and be prepared to work in Utah and have the ability to travel as needed.Military experience preferred but not required.Salary: To Be Determined Job Code: RNCaseMNGR_081812
