Job Summary
Respon! sible for abstracting, coding, and sequencing the classification of medical and surgical procedures, diagnosis, and treatment modalities on Inpatient and Day Surgery records. Selects the most accurate and descriptive codes from the listings of International Classification of Diseases, Ninth Revision, Clinical Modification, ( ICD -9-CM American Medical Association Current Procedural Terminology ( CPT -4) Coding system, and Healthcare Common Procedure Coding System ( HCPCS ). Assigns DRGâs and performs coding compliance reviews. Abstracts and codes pertinent medical data into multiple software programs.
Responsibilities
1. Responsible for the coding Multispecialty/Proficiency ( ICD -9 CM, CPT , HCPCS Coding) and Facility IP Coding
2. Ensures coding compliance within the Medical Record Policies and Procedures through on-going educational sessions with Medical Records. Follows Joint Commission, Medicare and third party documentation guidelines and t! he official guidelines for assigning ICD -9-CM diagnosis and p! rocedure codes in efforts to continually improve the quality of medical record documentation.
3. Responsible for maintaining current ICD -9 coding skills in keeping with regulatory changes.
4. Thoroughly reviews medical records and accurately codes the admitting (when needed), principal and necessary secondary diagnoses and procedures using ICD -9_CM coding conventions.
5. Extracts pertinent data from the patientâs health record, and determines appropriate coding for reports and billing documents. Identifies codes for reporting medical services, procedures performed by physicians, and enters codes into computer system.
6. Utilizes the coding query process to providers to request missing information. Corrects records with inconsistent information and/or containing discrepancies.
7. Perform routine audits independently and participates in performance improvement.
8. Ability to organize and lead teams. Prioritizes and coo! rdinates work processes. Ability to assist in conflict management of team. Provides support to supervisor and management.
9. Provides feedback and education to physician and professional staff regarding changes in coding methodology and enhanced documentation procedures for optimizing reimbursement.
Problem Solving
Challenges encountered by the incumbent include accurately coding information when available documentation is vague or ambiguous; identifying computer problems and working with computer technicians to make necessary repairs; and coordinating with the billing office to identify unbilled accounts and locating or generating the coding required to receive reimbursement. The incumbent resolves questions or requests from various entities that involve researching and verifying coding in patient records. This position has the authority to make decisions to optimize reimbursement within State and Federal laws. If assigned lead worker status, the! incumbent is authorized to schedule, direct, and oversee the work of H! ealth Information Coders and other subordinate department personnel. The incumbent must abide by Hospital, State, and Federal coding guidelines established by the Joint Commission, American Hospital Coding Association, and Medicare/Medicaid, etc
Minimum Qualifications
Required certifications include at least one of the following: Certified Coding Specialist ( CCS ), Registered Health Information Administrator ( RHIA ), or Registered Health Information Technician ( RHIT )
Must have an Associate Degree in related area.
Minimum 3 years to 4 years ongoing inpatient coding and abstracting experience in ICD -9-CM, DRGs, and CPT / HCPCS , including modifiers and APCs required
