To function as a spine/orthopedic services patient liaison that shall
facilitate continuity of care throughou! t hospitalization. To organize
and anticipate pre-admission and discharge planning needs. Conduct
data review to identify complication trends and implement
appropriate intervention. Consult with members of the
spine/orthopedic team including physicians to ensure the delivery of
quality care.
Coordinates admission of transfer patients or add-on emergent procedures with the OR, Lab, ICU, Nursing
Supervisor, etc.
Makes rounds on inhouse patients, documents assessments for surgeons or designee to utilize during their
daily rounds.
Provides education support to surgical patients and their families perioperatively. Uses a standardized
method of pre-op teaching.
Available to provide education to nursing staff or assist in care of the patient post-operatively.
Liaison with other hospital services to facilitate hospital discharge (case mgt., orthopedic department, etc! .)
Qualifications
Qualifications:
! RN State License required.
Bachelor's in Nursing preferred.
Two (2) years of clinical spine/orthopedic experience in a hospital setting required.
One (1) year experience with quality improvement data collection preferred.
Effective communication, organizational and basic computer skills.
