DUTIES:
OVERVIEW:
The LPN/LVN provides basic and skilled practical nursing care as assigned in a medical or surgical ambulatory care setting. Specialties represented in thi! s setting include: Allergy, Cardiology, Dermatology, ENT, General Surgery, Heart Failure/Transplant, Hematology/Oncology, Infectious Disease, Neurosurgery, Orthopedics, Orthopedic Hand, Orthopedic Spine, Podiatry, Rheumatology, Thoracic, Urology, Vascular, and Women's Health (Obstetric, Gynecology).

he LPN/LVN provides complex nursing care to a variety of acutely or chronically ill patients who require astute observations. The LPN/LVN is expected to be alert to subtle changes and to take appropriate and timely actions to alleviate or prevent problems. Understanding the nature of the illness and/or disease and treatment regimens is required to facilitate prompt reactions to subtle changes in the patient's condition.

The LPN/LVN varies the order and sequence of assigned procedures and nursing care based on the patient's condition and uses judgment in selecting the most appropriate guidelines to apply. Established, specific and detailed guidelines ! are available as resources. Other guidelines include the patient medica! l record, procedure records, plans of care, medical histories, etc.

Supervision of this position is by a registered nurse, who assigns patient and nursing care duties either verbally or in writing through general instruction. The LPN/LVN plans, schedules and performs nursing care, problem solves and modifies care independently, utilizing previous experience, policies/procedures, standards of care and standards of practice as a basis for decision making. Work may be spot-checked for completeness, technical soundness, appropriateness and compliance with facility policies/procedures.

Additionally, the LPN/LVN is professionally and clinically responsible to the Associate Director/Patient Care Services through the Nurse Manager, Specialty Clinics and ACNS Ambulatory Care and work in collaboration with a variety of providers to deliver patient care.

CLINIC RESPONSIBILITIES INCLUDE:
• Complete clinic intake (vitals, reminders)

• Mai! ntain clinic flow, room turnover

• Stocking, monitoring equipment and supplies for outdates

• Patient follow-up over telephone

• Administer medications based on scope of practice and hospital policy

• Set-up/take-down rooms for outpatient procedures (i.e. Cystoscopy, Prostate Biopsy, CMG, wound care, etc.)

• Assist providers and other nursing staff with minor procedures and outpatient recoveries

• Assist with coordinating cleaning/processing/ordering of instruments and scopes

• Facilitate communication between providers and patients/families

• Patient assessment

• Patient/family education

Work Schedule:
Part-Time, Monday to Friday, day shift.

The selected applicant will be assigned part time (0.2 FTE).

Monday through Friday to the Surgical Specialty Care Clinics, where they will float among different clinics based on staffing need, procedures, clin! ic schedules, etc. Orientation will be provided to all clinic areas.

Preferred Experience:
• Minimum 1-2 years recent medical/surgical LPN experience

• Experience in outpatient clinics, particularly surgical specialty clinics (i.e. Neurosurgery, General Surgery, Vascular, Orthopedics, Dermatology, ENT, etc.)

• IV certification preferred or obtained within 1 year of hire

Six months of qualifying experience as an LPN or LVN; or

Graduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing,! or patient care facility that provided a practical knowledge of human ! body structure and sterile techniques and procedures; or

Graduation from an approved school of at least 24 months duration.

GS-5

Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent.

Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems.

Demonstrated ability to serve as a responsible member of the nursing team and interact with patients, family members, professional and other supportive personnel.

Knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Observation and documentation will include patientÂ's response to medication administered and the reporting of any noted change in pat! ientÂ's condition to RN or MD/DO.

Knowledge and ability to recognize the need for and to institute emergency measures when indicated, promptly seek the assistance of the RN or MD/DO, and assist in resuscitation procedures in cardiac and/or pulmonary arrest.

Recognizes and appropriately responds to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action.

GS-6

Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent.

Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations.

Knowledge and ability to appropriately carry out assigned patient care based on the patientsÂ' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and re! cord relevant] patient information.

Ability to observe, identi! fy and respond to the patientÂ's needs for care, including medication, equipment-assisted care and patient/family education. In organizing and delivering care, the LPN/LVN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related factors.

Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures.

Knowledge and ability to recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed.

Knowledge and understanding of human behavior, patient motivations and reactions to situations, and ability to appropriately utilize this knowledge in working effectively with patients, family members, and other staff.

Establishes constructive relationships with individual patients and their families to elicit feeling! s and attitudes, and to promote positive relationships, communication and socialization skills. Fosters an environment of respect for individual patient and family rights to privacy and dignity in all aspects of care delivery. Effectively incorporates knowledge and understanding of established customer service standards in all interactions with patients, family members, and/or other internal/external customers.

Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patientÂ's condition before, during, and following the procedure. Serves as a preceptor in orienting, educating, and training less experienced LPNs/LVNs or NAs/HTs related to support duties for these more complex, specialized tests/procedures.

Physical Requirements:
The incumbent must be able to perform all activities of the position without restriction. This position involves considerable standing, walking, stooping, bending, reaching, lifting and turning. It requires repositioning patients and assisting with transferring patients to wheelchairs/stretchers. Mechanical devices when available are required to be used for heavy or difficult to lift patients. The work may involve pushing or lifting weight in excess of 50 pounds and going up and down flights of stairs. The position requires visual and auditory acuity at a level to provide safe and effective nursing care. The positions requires the nurse to work for periods of time with arms above shoulder level or with the neck in a fully flexed or extended position and the ability to physically control or protect him/herself from physically abusive patients.

The LPN/LVN m! ay be assigned to patient care on any tour of duty and relieves in othe! r areas as required. The work environment involves high risks and discomforts which require specific knowledge and safety practices in terms of, but not limited to (a) infection control, (b) contact with or disposal of secretions and/or drainage, (c) verbal and/or physical abuse from patients and/or visitors, and (d) working around heavy mobile equipment such as carts, X-ray machines, etc.
