Job Description

Department: Nursing
Reports To: Registered Nurse or Licensed Practical/Vocational Nurse

Exempt Status: Non! exempt

FUNCTION: Cares for Residents needing nursing care and complex medical care under the direction and supervision of a registered nurse or a licensed practical/vocational nurse.

QUALIFICATIONS:Minimum Qualifications:

  • Freedom from illegal use of drugs.
  • Freedom from use and effects of drugs and alcohol in the workplace.
  • Anyone found guilty by a court of law of abusing, neglecting or mistreating individuals in a health care related setting are ineligible for employment in the position.
Education and/or Experience: High school diploma or general education degree (GED), or three to six months related experience and/or training or equivalent combination of education and experience preferred.

Certificates, Licenses, Registrations: Graduate of a state approved nursing assistant training program and passed a state approved competency examination (written and skills) within 1! 20 days of employment; name entered into state nurse aide regi! stry.

LANGUAGE SKILLS: Must be able to express self adequately in written and oral communication and to communicate effectively in an interdisciplinary care setting with Residents, families and staff members.

MATHEMATICAL SKILLS: Ability to add and subtract three digit numbers, calculate percentage of whole, measure in cubic centimeters (cc) and measure in feet and inches. Ability to perform these operations using units of weight measurement, volume, length and height.

REASONING ABILITY: Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:Care and Services:
  • Takes and records vital signs.
  • Measures and records height and weight. Calculates and records weight gain or loss.
  • Calculates and records fluid intake and output.
  • Cares for the Resident’s environment.
  • Recognizes abnormal changes in body functioning and the importance of reporting such changes to a supervisor.
  • Cares for Residents when death is imminent.
  • Makes routine rounds on assigned Resident
  • Bathes Residents.
  • Assists with grooming, including mouth care.
  • Dresses Residents.
  • Toilets Residents.
  • Assists with eating and hydration.
  • Uses proper feeding techniques.
  • Calculates and records percentage of meal eaten.
  • Observes Resident’s skin when giving care and reports changes to licensed nurse.
  • Provides skin care.
  • Assist the Resident to transfer, position and turn.
  • Attempts to meet Residents’ mental health and social needs.
  • Modifies own behavior in response to Residents’ behaviors.
  • Is aware of developmental tasks associated with the aging process.
  • Responds to Residents’ behaviors in ways to help minimize the behaviors .
  • Allows Residents to make personal choices.
  • Provides and reinf! orces other behavior consistent with the Residents’ dignity.
  • Uses the Residents’ families as a source of emotional support.
  • Provides nursing rehabilitation/restorative nursing by:
Promoting Resident’s/Resident’s ability to adopt and adjust to living as independently as possible.
Training the Resident in self care according to the Resident’s abilities.
Training the Resident in the use of assistive devices for transferring, walking and mobility, dressing and grooming, eating and swallowing.
Maintaining the range of motion; turning and positioning in bed and chair.
Providing bladder and bowel retraining.
Instructing the Resident on the care and use of prosthetic and orthotic devices.
Incorporating training and practice activities which include repetition, physical or verbal cueing and task segmentation.
Cares for Residents with complex medical or nursing needs, such as those who are:
  • dependent upon! medical equipment such as infusion therapy, enteral feeding pumps,
  • Recuperating from surgery.
  • Receiving intensive rehabilitation therapy.
  • Head injury Residents.
  • Cardiovascular Residents.
  • Respiratory care Residents.
  • Oncology Residents.
  • Residents with chronic wounds.
  • Confused or cognitively impaired.
Provides non-Resident care including, but not limited to;
Washing wheelchairs.
Cleaning soiled utility areas.
Cleaning nurses station.
Cleaning shower and whirlpool rooms.
Cleaning kitchen areas on the nursing unit.

RESIDENT RIGHTS: Promotes residents’ rights, assists residents to make informed decisions, treats residents with dignity and respect, reports suspected abuse or neglect, and supports independent expression, choice and decision-making.

LEADERSHIP: Demonstrates willingness to try new tasks, generates new ideas for change, evaluates an! d recognizes priorities, challenges others to learn, keeps current and ! integrates new information, communicates and models organization values and fosters high performance.

INFECTION CONTROL: Applies hand washing principles during daily work, demonstrates understanding of isolation precautions, recognizes signs and symptoms of infection and complies with the employee health program, demonstrates understanding of the process for identifying and handling infectious waste, maintains personal hygiene, complies with OSHA standards in the work place and demon! strates understanding of cross-contamination.

INFORMATION MANAGEMENT: Responsible to document cares provided, promotes confidentiality of resident information, reports suspected violation of security/confidentiality issues.

INTERPERSONAL SKILLS: Demonstrates active listening techniques, gains support through effective relationships, treats others with dignity and respect and seeks feedback

CONTINUING EDUCATION: Attends in-service and education programs and attends continuing education required for maintenance of professional certification or licensure.

PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is frequently required to stand, walk, use hands or fingers, reach with hands and arms, talk or hear. The employee is occasionally requi! red to sit, climb or balance and stoop or kneel. The employee on occasi! on will assist to transfer residents of varied weights and sizes utilizing mechanical lifts, and recommended transfer techniques. The employee will follow protocols related to use of lifts. Specific vision abilities required by this job include close vision, distance vision and peripheral vision.

WORK ENVIRONMENT: The work environment characteristics described are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee is occasionally exposed to blood or other body fluids, fumes or airborne particles and toxic or caustic chemicals. The noise level in the work environment is usually moderate.
